Class Drop Request Form

Class drop requests go into effect immediately.  If you wish to complete the remaining classes in the current month, do not submit your form until you come in for your last class.  No credits or refunds are given for remaining classes if you drop mid month.  You may submit your form by faxing it to 810-229-4998, dropping it in the drop box by the front desk, or mailing it to 12420 E. Grand River, Brighton, MI  48116.  (We must receive it before the 1st of the month when payments are processed.)  We do not accept email or telephone drop requests or verbal drop requests at the desk.

Registration Form

All students enrolled in a class must have a registration form signed by a parent.  All those participating in open gyms, birthday parties, field trips and other special events must have a registration form signed by a parent.
